Personal data are processed in the seat of the Administrator of Personal Data and in the enterprises of the medical entity entered in the Register of Medical Entities of the Mazovian Voivode under number 000000007431.
Personal data will be processed for purposes related to the provision of health care services or other services for health care purposes, including scheduling an appointment, as well as for the protection of patient rights and the fulfilment of obligations incumbent on the Administrator under applicable laws.
The Controller processes your Personal Data only to the extent and for the purpose of performing the services in accordance with: (i) Article 6(1)(b), (c), (d) of the General Data Protection Regulation; (ii) Article 9(2)(h) of the General Data Protection Regulation; (iii) the Act of 15 April 2011 on medical activity (i.e. Journal of Laws of 2018, item 160 as amended); (iv) the Act of 27 August 2004 on health care services financed from public funds (i.e. Journal of Laws of 2017, item 1938 as amended); (v) the Act of 6 November 2008 on Patient Rights and Patient Ombudsman (i.e. Journal of Laws of 2017, item 1318 as amended); (vi) the Regulation of the Minister of Health of 9 November 2015 on types, scope and models of medical records and the manner of their processing (Journal of Laws of 2015, item 2069 as amended); (vii) the Act of 27 June 1997 on Occupational Medicine Service (i.e. Journal of Laws of 2014, item 1184 as amended).
If necessary, the data may be disclosed to entities processing personal data authorised or commissioned by the Administrator and to other entities authorised by law, including the NFZ.
Personal data will be stored in accordance with the legal regulations in this respect, until the time required by separate applicable laws in this respect.
Personal data is not profiled, nor is it transferred to a third country or international organisation.
If personal data has been provided with consent, there is a right to withdraw consent at any time, without affecting the compatibility of the processing carried out on the basis of consent before its withdrawal.
You have the right to access and rectify your data, to erase or restrict its processing, the right to object to its processing and the right to data portability.
You also have the right to lodge a complaint in relation to the processing of the aforementioned data with the data protection authority.
